Frequently Asked Questions

  • We are currently in-network with HealthChoice and also accept fee-for-service payment.

For all other PPO insurance plans, our practice is considered out of network. If you would like to seek reimbursement, please contact your insurance provider to verify your out-of-network mental health benefits and any documentation they may require.

  • If you are covered by HealthChoice, your visits will be billed in-network and are subject to any applicable copays, coinsurance, or deductibles determined by your plan.

For patients using fee-for-service, the rates are:

    • Psychiatric Diagnostic Evaluation (50 minutes): $300

    • Medication Management Follow-Up (25 minutes): $150

If you have a different PPO plan, we are considered out of network. You would pay the fee-for-service rate at the time of your appointment and may submit the receipt to your insurance provider to seek reimbursement based on your out-of-network mental health benefits.
